  1. What is the free consultation?

    A free consultation is provided via phone in order to determine if On Par will meet the needs of your child and his/her goals.

  2. What to expect at initial evaluation?

    This is a 60 minute visit where families will come away with an individualized plan for treatment to address the child's needs and family goals.

  3. Are you in network with insurance?

    At this time, On Par Pediatric Therapy does not participate with commercial insurance plans. By eliminating the constraints of insurance we are able to address your child's needs, achieve family focused goals, and spend the proper time supporting the child and the family through the process of physical therapy.

  4. What is the cost of services?

    Rates vary depending on services rendered.

  5. What forms of payment are accepted?

    Cash, check, or credit card payments. Payments can be made through the patient portal (click Sign In at top of page) or in person at time of service.

  6. What is a Superbill?

    Although On Par Pediatric Therapy does not bill insurance, families are provided with a Superbill upon request. This is a detailed invoice of services rendered, including appropriate ICD and CPT codes for each service. Families can submit the Superbill to their insurance provider for possible reimbursement. While the reimbursement is not guaranteed and depends on the individual's insurance plan and coverage, the Superbill serves as documentation to support claims of out of pocket expenses.
